Any loss or theft of your passport should be reported on the LS01 form, which you will have to complete in order to obtain a replacement passport. Please complete the LS01 form [PDF, 795 KB] in addition to the relevant passport application form.

The following documents will be required when applying for a replacement of a lost or stolen passport:

• completed application form C1/C2
• two passport sized photographs
• police report about the lost/stolen passport
• a form of ID eg driving licence etc.
• the fee for a new passport (Passport fee table).

If you have a copy of the lost/stolen passport please present this at the time of the application.
